Kansas Medical Resource contracts with physicians and other health care providers. Physicians must have a full license issued by the Kansas Board of Healing Arts, a DEA Certificate, and malpractice insurance. Each provider also must agree to have a complete background check.
To register as a KMR provider, an eligible physician must complete the appropriate form below. Resident physicians must comply with all University of Kansas Medical Center policies and procedures regarding participation in the KMR program.
